Why should I enroll in the Hyatt Legal Plan?
The Hyatt Legal Plan gives you and your family access to professional legal representation at an affordable price. It’s like having your own lawyer on retainer. When you use a plan attorney (one of the 10,000 attorneys who are members of the Hyatt Legal Plan network), the plan:

Pays all attorney fees for covered services—in full
With no deductibles or copays, and
With no claim forms.

Do I have to use a plan attorney?
No. You can use any attorney you wish. However, if you use an attorney who is not a member of the Hyatt Legal Plan network, you will need to file a claim for reimbursement of your attorney’s fees and the benefits you receive will be based on a set fee schedule.

How can I enroll in the plan?
If you are a newly-hired benefits-eligible associate, you will be able to enroll in the Hyatt Legal Plan during your initial enrollment period. Otherwise, you may enroll in the Hyatt Legal Plan during any annual Benefits enrollment—as long as you are a benefits-eligible associate.

NOTE: Enrollment in the Hyatt Legal Plan is available only to benefits-eligible associates of Advocate Health Care.

How do I use the plan?
Whenever you need legal assistance, all you need to do is contact the Hyatt Legal Plan:

Online—go to www.legalplans.com any time, day or night, or
By phone—call 800.821.6400 between 7 a.m. and 6 p.m. CT, Monday-Friday.

You will receive the name, address and phone number of plan attorneys in your area, as well as a case number. You can contact any one of these attorneys to schedule an appointment. And remember…as long as you see a plan attorney, there’s no deductible or copay, no waiting period, no claim forms and no limits on usage for covered services.

What services does the plan cover?
The Hyatt Legal Plan provides legal assistance in the following areas:

Consumer Protection
Debt Matters
Defense of Civil Lawsuits
Document Preparation
Family Law
Immigration Assistance
Real Estate Matters
Traffic Matters/Criminal – excluding DUI
Wills and Estate Planning

Are there any legal services that the plan does not cover?
Yes. The plan does not provide any benefits (including consultations) related to the following:

Employment-related matters, including company or statutory benefits
Matters involving the employer, MetLife® and affiliates, and plan attorneys
Matters in which there is a conflict of interest between the employee and spouse/domestic partner or dependents, in which case services are excluded for the spouse/domestic partner and dependents
Appeals and class actions
Farm and business matters, including rental issues when the participant is the landlord
Patent, trademark and copyright matters
Costs and fines
Frivolous or unethical matters
Matters for which an attorney-client relationship exists prior to the participant becoming eligible for plan benefits

What happens if I leave Advocate?
If you are enrolled in the Hyatt Legal Plan and you retire or your employment terminates, you can "port" the legal plan. To do so, you must contact Hyatt Legal Plans' Client Service Center—800.821.6400—within 30 days of your termination or retirement date and request to continue coverage or to port the plan. You prepay Hyatt Legal Plans directly in a lump sum for the full amount due for the portability period (currently a set period of 30 months). The covered services and Hyatt's coverage of you, your eligible spouse and your eligible dependents will be the same as coverage for the group legal plan for that period.
