What is Healthe You and why does it exist?
At Advocate, we are committed to providing you and your spouse or partner with information and
resources that support you and your health. That is why we implemented the easy and confidential
Succeed Questionnaire, developed by HealthMedia®. This Health Questionnaire develops a tailored
personalized action plan based on where you are, and most important, where you want to be.
Making healthy lifestyle changes will not only help you live longer, but healthier and happier too.
Participating in the Succeed Questionnaire will open up opportunities to participate in online
programs, have access to interactive tools and resources, as well as downloadable wellness and
medical libraries. These resources are designed to help you reach the goals you have set for yourself.

Questions about Healthe You
Who can participate in the Healthe You program?
The Healthe You program is only available to associates and spouses/domestic partners who
elect one of Advocate's health plans. As you review the questions and answers below, keep in
mind that when referring to "you", your covered spouse/domestic partner must also participate in
the Healthe You program to receive the Healthe Reward bonuses.
As a new hire, am I required to take the Succeed Questionnaire?
Succeed is part of the health plan so it is strongly encouraged. Should you and your spouse elect
not to participate, you will be limited to the $1000 deductible medical plan.
What is included in the Healthe You program?
Financial incentives known as Healthe Rewards, interactive tools, downloadable resources and online programs to help you maintain a healthy lifestyle, are just the start of the benefits of Healthe You. The online tools include the Succeed Questionnaire.
What is a Healthe Reward?
Healthe Rewards are earned for doing good things. One of the ways to receive this reward is if
you are smoke free, or have completed the 6 week tobacco cessation program through
Nationwide Better Health. Your tobacco status is captured through the Succeed Questionnaire.
How do I know the Healthe You website is safe?
This
website uses secure socket layer (SSL) encryption to protect the transmission
of information. This means that
when you submit information to this site, you will be using secure online
forms. All the information
submitted through these forms is stored securely offline where others cannot
access it.
Know Your Numbers
Knowing your biometric numbers, such as total cholesterol, triglycerides and glucose is an important step in being the healthiest you! Site-based screenings will be available annually in the fall.
Can I still take the questionnaire if I don’t know my numbers or if I know some of my numbers but not all of them?
As a new hire – you will have the opportunity to capture these numbers through your employment physical. Be sure to keep this information until you are able to participate in the questionnaire. If you are not a new hire, supply the numbers that you know, as long as they are less than one year old, and check “not sure” for others. Having your numbers available will provide you with a more detailed personalized action plan.
If I participate in a site-based screening, when will results be available?
Your
results will be provided to you immediately. You
will want to keep the printed record that is provided so that you may enter
your results in the Succeed Questionnaire. There
is a handy wallet card available for you to download on the Healthe
You website:

This
card will allow you to copy your numbers and carry them for your convenience or
place with your medical records.
Tobacco-Free Incentive
I am tobacco free. How can I apply
to get the tobacco-free bonus?
If you participate in the health plan, you can take the Succeed Questionnaire. You will report your tobacco status on this questionnaire. Once your information is captured, the bonus, if applicable, will appear on your Benefits Confirmation Statement and be reflected within your first, or second paycheck.
My spouse is covered by the health plan, but is a smoker. Can
I still get a tobacco-free bonus?
You will be rewarded for reporting your tobacco-free status if you are tobacco free. As long as you both complete Succeed.
What if I or my spouse decide to become tobacco free now or at some point in the future?
In order for you to receive the tobacco-free bonus you and/or your spouse will need to be tobacco free for the last 6 months. You and/or your spouse may qualify for the bonus if you are actively enrolled in the smoking cessation program through Nationwide Better Health. You may report your smoke free status on the Succeed Questionnaire after approval through the coach. For those of you who have previously reported tobacco use during last fall’s enrollment, you will be eligible to revise your tobacco status, but only through your Nationwide Better Health coach.
What
happens if I or my spouse report that we are tobacco free and we are not?
If
it is determined that the tobacco-free status was misreported, any one or
several actions may occur:
1.
Response would
be viewed as a falsification of records which warrants disciplinary action up
to and including dismissal of employment per Advocate policy
2.
You and your
spouse would risk expulsion from the health plan
3. Any
Healthe Rewards received will be repaid automatically through payroll
deduction
Healthe You and Your Spouse/Domestic Partner
Is the Succeed Questionnaire only available to me and my spouse/domestic partner during my Benefits Enrollment Period?
No, you may access the Succeed Questionnaire year round if you have not yet participated. You may not, however, go back and change previous answers until fall open enrollment.
Can I and my spouse take the Succeed Questionnaire at different times during my enrollment period?
Yes, however, you as the associate will need to give permission for your spouse to have access to complete the questionnaire.
Can my spouse access Succeed from work or home?
Yes – Through the following website, your spouse can access Succeed 24 hours, 7 days a week:

Since the Succeed Questionnaire is hosted on Advocate InfoExpress, does this mean my spouse can see all my other benefits records?
No, your spouse will have a unique User ID and Password that will only permit him or her access to the Succeed Questionnaire.
If my spouse is covered by the plan but does not want to take the Succeed Questionnaire, will I be eligible for any bonuses?
No, in order to earn any bonus, if you and your spouse are covered, both must complete the Succeed Questionnaire in order for any bonus to be payable.
You have indicated my spouse has a unique User ID and Password, how will I receive this information?
Once you consent for your spouse to participate, his or her User ID and Password will appear on your screen. For convenience, the spouse’s User ID will be first name_last name as it appears on your dependent file in your benefit record. The spouse’s Password will be an 8-digit number representing his or her date of birth (mmddyyyy). Your spouse will have the ability to change his or her Password if he or she desires to do so?this ensures everyone’s privacy.

How do I get to the Succeed Questionnaire?
1. Logon to http://www.AdvocateInfoExpress.com 2. Click on “Health and Welfare Benefits” 3. Log in 4. Click on “Complete Succeed”
How much time should I allow to take the questionnaire?
It will vary by individual, but it usually takes between 30-40 minutes to complete.
If I start the Succeed Questionnaire, but do not have time to complete it, can I go back in to finish answering the questions?
You can leave and return to the questionnaire to complete it as many times as you want. Each time you click "next", all the previous responses are saved. Completion of the Succeed Questionnaire occurs only when you have clicked the "submit" button at the end of the questionnaire.
What happens if I don’t complete my questionnaire?
Throughout the questionnaire, there are certain questions that are required. You will need to work through the entire questionnaire to ensure all these questions are answered. You will be reminded before you submit the questionnaire if any questions have been missed.
What if I do not have an email address?
Google offers an email account that is free. In order to get a G-mail account, follow these steps:
1. Visit http://www.google.com 2. Click on the Gmail link on the upper left corner 3. Click “Create your account” 4. Complete personal information 5. Click “Accept” after you’ve read the terms of service 6. Click “I’m ready, show me my account”
How will I get the results of the questionnaire?
You are strongly encouraged to provide an email address so that information can be sent to you. If not, you have access to your results from your personal Healthe You page.
So, I’ve completed my questionnaire and I received a Healthe Action Plan. The plan recommended several programs that might benefit me. How do I look at the Healthe You programs in the future?
For your convenience, you can always use Advocate InfoExpress to serve as a quick link to your personal Healthe You page and your reports so that your information is maintained confidentially. Your spouse will also have access at any time using his or her User ID and Password.
