Secondary Benefits

The plan’s secondary benefits provide additional protection if—within any 12 month period—you use up an available primary benefit. This secondary benefits feature provides up to a 45% discount on subsequent purchases at an unlimited frequency. These secondary benefits are summarized below.

1 Member cost is $15 higher in Alaska, California, Hawaii, Oregon and Washington.
2 The cost of premium progressive lenses equals the basic progressive lens retail price plus 80% of the balance over this price.

Additional purchases and out-of-pocket discounts
If you elect this coverage, you will receive a 20% discount on remaining balances at participating providers beyond plan coverage. This discount will not apply to the professional service fees of EyeMed providers or the cost of disposable contact lenses. Benefits may not be combined with any discount, promotional offering or other group benefit plans. Allowances are one-time use benefits; no remaining balance. Lost or broken materials are not covered.
