To qualify for coverage under an Advocate plan, your dependent(s) age 19-25 must be enrolled in the minimum number of hours required by the accredited learning institution for full-time student status. Upon attainment of your dependents 19th birthday, you will be asked to provide initial proof of Student Status.

Thereafter, each year (or whenever there is a question for claims processing) Advocate InfoExpress will ask you to update information regarding verification of full-time student status for your dependents.

A completed student verification form and necessary paperwork such as an official class schedule or proof of registration must be mailed or faxed in when requested, prior to the deadline, to ensure eligible students are covered under your Advocate plan. In order to get claims paid in a timely manner, it is important to fax or return this form promptly.

If your child no longer qualifies for full-time student status (no longer carries minimum number of hours required for full-time status, graduates), it is the associate's responsibility to notify Advocate InfoExpress to drop the dependent. If the associate does not provide the timely notice—even if the associate continues to pay contributions for the child's coverage—claims will be denied if student status is questioned and found not to exist and a refund of any excess contributions will not be provided.
