your employment with Advocate Health Care.
Exit Interview
Advocate encourages you to complete an exit interview. Please ask your Manager or
contact the Human Resources department at your site for a copy of the survey. A
self-addressed, stamped envelope will be provided for you to confidentially
return your response.

What if my address changes?
If your address changes in the future, please send written notification to
Associate Records. Please include the last four digits of your Social Security number, phone number,

I am retiring. What should I do?
When you retire, you may need to contact Medicare and the Social Security
Administration to get information about these benefits.
To get information about your Advocate Retirement Program benefits, contact
Advocate InfoExpress. You may want to contact an Advocate InfoExpress
benefit service representative at 800-775-4784 in advance of your retirement date to get
information and start the retirement process.
How will I get my last paycheck?
Check with your Manager.
What if my separation is due to work-force reduction?
If you're affected by work-force reduction and you're eligible for severance,
please contact your site Human Resource department for detailed information
about benefits continuation, unemployment and severance arrangements.
What happens if I am re-employed by Advocate?
If you end your employment with Advocate and then return to Advocate in a
benefits-eligible status, you'll be eligible to make a new election for
Medical and Dental coverage and to participate in a Health Care FSA. You'll be
eligible to
make a new election to a Dependent Day Care FSA if your rehire date is in a
new calendar year after your termination date. There will be a 30-day waiting
period before your participation will become effective.
If you end your employment and then return to Advocate within one year (365 days)
in a PTO-eligible status, your original PTO
eligibility date will be in effect for PTO accrual purposes.
For the Retirement Program, if you're re-employed by Advocate, the IRS
break-in-service guidelines apply. Please refer to the
Retirement Program summary for more detailed information.
Benefits information
Paid Time Off
Unused paid time off (PTO) hours will be paid with your final paycheck (after
your separation notice has been received by Associate Records). Payout of PTO
hours is authorized by your Manager.
Continuation of Benefits
In most cases, if you're currently enrolled in a Medical or Dental plan
your coverage will continue until the end of the month in
which you work your
last day. Written notification of health care coverage continuation will be
sent to you using first class mail. Be sure that your Manager submits your
separation information in Manager's Desktop, and notes your date of separation to
ensure timely delivery of health care coverage continuation information.

Basic Life Insurance
Basic Life Insurance is a benefit offered by Advocate to active,
benefits-eligible associates. This benefit covers benefits-eligible associates
through the last day worked. If you were eligible for Basic Life Insurance,
conversion options are available to you. A conversion request form must be
completed and returned within 31 days from your last day worked.

Optional Life Insurance
If you're currently enrolled in Optional Life Insurance, you are eligible to
continue your coverage after your last day of work.

You may continue to participate in the credit union. If you have any questions,

Retirement Program
You will become eligible for a distribution for any vested portion of the
Retirement Savings Plan and Portable Pension Plan after you receive your final
paycheck and PTO payout. If you remain employed in any capacity, including PRN,
float, on-call or registry positions, you aren't eligible for Retirement
Program distributions since you may be eligible for future accruals.
You may access your Advocate InfoExpress account by phone or by logging onto
the site. Either way, you'll need to have your Social Security number and your
personal identification number (PIN) available.
Distribution information will be mailed to you after you receive your final
paycheck and your separation date is received by Advocate InfoExpress. The
packet will describe the distribution process and available distribution
options. A Retirement Savings Plan distribution is available 30 days after your
termination date by calling Advocate InfoExpress and speaking with a benefit
service representative. The Portable Pension Plan distribution process is a
two-step process and can take approximately 90 days:

You will need to apply for your pension benefit using the
Portable Pension Plan Application Form (available on
Advocate InfoExpress).
You'll need to include documentation of
your birth and, if you are married, your spouse's birth when you return
the form. |

A benefit quote and the necessary election forms will be sent to
you.

Return of property
Property of Advocate such as an ID badge, pager, cell phone, computer equipment, library
materials, keys or uniforms must be returned immediately to your site. Contact
your Manager or site security to return property.
Unemployment
Unemployment insurance is a state-operated program. If you meet the legal
requirements, you may be entitled to benefit payments while you're unemployed.
For more information, you may call the Illinois Department of Employment at
888-FOR-IETC (888-367-4382). If you would like to locate the unemployment office nearest to
you, please be ready to provide your ZIP Code.
Employment verification
For employment verification, the verifier should use the web-site at
www.verify.theworknumber.com or by phone at 800-367-2884. The verifier should follow the menu to select
the correct category and will need to provide the following information in
order to access associate information:

Advocate's company code (10134), and |

Associate's Social Security number.

If the verifier is unable to access the web-site, the verifier should call the
customer service center for "The Work Number for Everyone" at 800-996-7566.
