Dentist Program (PDP) |

MetLife PDP allows you to chose any dentist you want for your dental care
needs. There are deductibles and coinsurance amounts to pay, and claim forms to
file. You’ll be reimbursed when the Dental Plan Administrator receives the
dental claim form from the dentist. Claims will be considered if the dental
expense is incurred while you are covered under the plan.

When you use a MetLife PDP participating dentist: |

When you choose your dentist from the list of MetLife PDP participating
providers, you receive maximum benefits and reduce your out-of-pocket
costs. MetLife PDP participating providers have agreed to discount
their fees and this savings is passed along to you, reducing your out-of-pocket
costs. Reimbursements are based on a negotiated fee schedule for participating
providers after the deductible is met. These negotiated fees are typically
discounted 20–30% from usual and customary charges. |

When you use a non-participating dentist: |

When you use a dentist who is not a MetLife PDP participant, your benefits are
lower and your out-of-pocket costs will be higher. For non-participating
providers, reimbursement is based on reasonable and customary charges. When you
use non-participating providers for your dental care, the plan covers expenses
as a percentage of the reasonable and customary charges according to the plan’s
reimbursement schedule. |
