Refills
For your convenience in refilling mail service orders, you have the following options:

1.   Use the Caremark Web site—www.caremark.com
2.   Use telephone reordering—(800) 966-5772 (TTY Assistance—800.863.5488
3.   Use a Caremark mail service order form. (Follow the instructions on the form included with your medication.)


ID Cards
There are two versions of the ID card that you must present to your pharmacist each time you order a prescription; to see a copy of these cards, click on the appropriate link below.

Caremark ID Card CVS/Caremark ID Card

Either card is valid; if you currently have a Caremark ID card, you may continue to use that card.

Note: If you are enrolled in the Caremark system and you need to purchase prescription drugs but have not received your Caremark ID card, you can simply pay the full cost for your prescription and then submit a Caremark claim form for reimbursement.
