Important! If you elect coverage under the High-Deductible Health Plan, these pharmacy benefits do not apply to you. The HDHP treats prescription drug costs like any other covered expense which makes them subject to the annual deductible.

Refills
For your convenience in refilling mail service orders, you have the following options:

1.   Use the Caremark Web site—www.caremark.com
2.   Use telephone reordering—800-966-5772 (TTY Assistance—800-863-5488)
3.   Use a Caremark mail service order form. (Follow the instructions on the form included with your medication.)

Either card is valid; if you currently have a Caremark ID card, you may continue to use that card.
