Advocate’s Integrated Health Advocacy ProgramSM—IHAP®—is
a holistic team approach to health care designed to help you and your
dependents cope with challenging or continuing health problems by partnering
with you to improve your overall health and life status.
Important! IHAP will identify associates and/or family members who may
benefit from participation in the program. If you or a covered family member
meets the criteria for participation, you will be notified by mail. You can
nominate yourself or a family member for participation in the program, subject
to approval by IHAP. Participation in the
program is completely voluntary and strictly confidential.
The IHAP Team is an integrated multi-disciplinary group of experienced health
care professionals that includes a:

Primary Advocate who will guide you—as a personal health
coach—through the IHAP process, review program-related information with you and
answer any questions you may have.

Behavioral Advocate who will review your life and family
history and will identify—and help you and your family cope with—any stress
factors in life.
|
 |
Medical Advocate who will conduct a thorough physical exam
and review personal health history, including available health records.
|
The members of your IHAP team will apply their combined skills to evaluate your
condition and—working with your current providers—develop a plan to treat you
as a whole person by taking into consideration all of your health-related
needs.
IHAP’s goal is to facilitate the healing process and promote optimum long term
health. In this way, it presents a unique opportunity for Advocate to support
your needs when you are coping with serious health problems while, at the same
time, assisting Advocate in maintaining an affordable health care benefit
program by keeping associates and family members who participate in the program
as healthy as possible.
