Important! The DHMO coverage option is not available to associates working at BroMenn locations.
The Dental HMO is simple to use. You just select a dental office from the dental provider listing and note the location when you enroll. Once your coverage is effective, you may call that dental office to make an appointment. You and your enrolled family members may each choose different general dentistry practices depending on your individual needs.
Preventive and diagnostic services such as exams, x-rays and cleanings are covered at 100% to encourage good oral health. For services covered at less than 100%, your copayment is based on a copayment schedule all participating dentists have agreed to accept. You pay your dentist this copayment at the time you receive the care. There are no claims forms to submit.
To request a dental provider listing, contact the plan administrator, FirstCommonwealth—by phone at 800.775.2246 (8 a.m. to 8 p.m. ct, Monday - Friday), or online at http://www.guardianlife.com/ click on "Provider Online Search" on right hand side under "Resources". Select "find a Dentist", select "DHMO/Pre-paid" for select a plan. Put in your zip code or address in where requested. Use "First Commonwealth DHMO" when selecting a network to search.
