Welcome to the Advocate Benefits Pages home page. If you want detailed
information about your benefits at Advocate, you’re in the right place!
At Advocate Health Care, we know that—as an Advocate associate—you are one of
our most important resources. With this in mind, it’s our responsibility to
offer you a comprehensive benefits program that includes a variety of options
and choices to meet your individual needs.
On these pages, there are several ways you may access information about your
benefits:

To get specific information about each Advocate benefit, simply
click on the item in the column to the left. |

For a brief description of each benefit plan, click on Summary at left. |

For phone numbers, addresses and Internet links to the organizations which
provide benefit services for Advocate, click on Contacts at left. |

If you want to return to the top of the page you’re currently on, just find and
click on a red arrow that looks like this: . |

If you want to print the page you’re currently on, click on the light blue
"print" tab just below the AdvocateOnline navigation menu. |
If you want to do more than read—for example, if you want to enroll or make
changes to your benefit choices or update dependent information—click on Advocate
InfoExpress at left. Advocate InfoExpress is easy to use and totally
secure. You need to know your Social Security Number and Personal
Identification Number (PIN) to access your personal account information. If you
don't know or can't remember your PIN:

Log on to Advocate InfoExpress
and click on the Retirement Benefits Logon. Follow the prompts to request a
PIN, or |

Phone Advocate InfoExpress at
(800) 775-4784 and use the Interactive Voice Response (IVR) system on the
Retirement menu to request a PIN. |
